Liver Function

The liver plays a central role in metabolism, detoxification, nutrient processing, hormone regulation, bile production, and energy homeostasis. Because of its broad physiological responsibilities, abnormal liver markers do not always indicate liver disease and should always be interpreted within the wider clinical picture.

Many common metabolic conditions—including insulin resistance, obesity, metabolic dysfunction-associated steatotic liver disease (MASLD), alcohol consumption, medications, infections, and strenuous exercise—may influence liver test results.

Important

Individual liver markers reflect different physiological processes. Meaningful interpretation depends on evaluating patterns across multiple tests rather than focusing on a single abnormal result.

Alanine Aminotransferase (ALT)

 

What it reflects

ALT is an enzyme found primarily within liver cells. Elevated blood levels usually indicate liver cell injury or increased hepatocellular stress.

Why it matters

ALT is one of the most commonly used markers for detecting liver inflammation and metabolic liver dysfunction.

Typical laboratory reference range

Generally below 40 U/L, although reference ranges vary between laboratories.

 

Generally favourable for metabolic health

Often below 25–30 U/L, when interpreted alongside the remainder of the liver profile.

Persistent mild ALT elevation frequently reflects metabolic dysfunction rather than acute liver disease. Insulin resistance, excess visceral fat, and metabolic dysfunction-associated steatotic liver disease (MASLD) are among the most common causes.

Aspartate Aminotransferase (AST)

 

What it reflects

 

AST is an enzyme found in the liver, skeletal muscle, heart, kidneys, and several other tissues.

 

Why it matters

 

Because AST is present in multiple organs, elevated values are less liver-specific than ALT and should always be interpreted in context.

Typical laboratory reference range

Generally below 40 U/L.

 

Generally favourable for metabolic health

Often below 25–30 U/L, provided no recent strenuous exercise or muscle injury has occurred.

AST is most informative when interpreted alongside ALT. The relationship between these enzymes may provide additional clues regarding metabolic liver dysfunction, alcohol-related injury, muscle damage, or other physiological processes.

Gamma-Glutamyl Transferase (GGT)

 

What it reflects

GGT is an enzyme involved in glutathione metabolism and bile transport.

Why it matters

GGT is highly sensitive to liver stress, bile duct dysfunction, alcohol exposure, oxidative stress, and metabolic disease.

Reference values

 

Typical laboratory reference range

Generally below 60 U/L, depending on age, sex, and laboratory.

Generally favourable for metabolic health

Often below 20–30 U/L, particularly when considered alongside other metabolic markers.

 

Integra perspective

Even when remaining within laboratory reference ranges, higher GGT values may reflect increased oxidative stress or early metabolic dysfunction. It is one of the more useful markers for monitoring improvements in metabolic health over time.

Alkaline Phosphatase (ALP)

 

What it reflects

ALP is an enzyme primarily produced by the liver, bile ducts, and bones.

Why it matters

Elevated ALP may reflect impaired bile flow, liver disease, or increased bone turnover.

Typical laboratory reference range

Generally 30–120 U/L, depending on laboratory methodology.

Integra perspective

ALP should never be interpreted alone. Combining ALP with GGT often helps distinguish liver-related causes from those originating in bone metabolism.

Bilirubin

 

What it reflects

Bilirubin is produced during the normal breakdown of red blood cells and processed by the liver before excretion.

Why it matters

Abnormal bilirubin may indicate altered liver processing, bile flow obstruction, or increased red blood cell breakdown.

Typical laboratory reference range

Generally below 20 µmol/L (approximately below 1.2 mg/dL).

Mild isolated bilirubin elevation is often benign, particularly in individuals with Gilbert's syndrome. Interpretation should always consider liver enzymes, symptoms, and the complete clinical picture.

Albumin

 

What it reflects

Albumin is the principal protein produced by the liver and plays an essential role in maintaining fluid balance, transporting hormones, nutrients, and medications.

Why it matters

Albumin provides insight into the liver's protein synthesis capacity as well as overall nutritional and physiological status.

Reference values

 

Typical laboratory reference range

Generally 35–50 g/L.

Generally favourable for metabolic health

Values comfortably within the laboratory reference range are generally consistent with healthy liver synthetic function.

Integra perspective

Low albumin does not necessarily indicate liver disease. Nutritional status, chronic inflammation, kidney disorders, gastrointestinal conditions, and systemic illness should also be considered.

• Total protein

• Bilirubin

• Liver enzymes

• Kidney function

 

Looking at the Whole Picture

 

No single liver marker provides a complete assessment of liver health.

For example:

  • Mild ALT elevation may reflect metabolic dysfunction rather than permanent liver injury.

  • AST may rise following strenuous exercise without indicating liver disease.

  • Elevated GGT may identify early metabolic stress before other liver markers become abnormal.

  • Increased ALP may originate from the liver or bone and requires additional interpretation.

  • Mild bilirubin elevation may be entirely benign in otherwise healthy individuals.

  • Albumin reflects liver synthetic function but is also influenced by nutrition, inflammation, kidney health, and chronic illness.

 

Patterns across the complete liver profile provide considerably more clinical value than isolated results.

Core Principle

 

The liver is central to metabolic health, nutrient processing, detoxification, and physiological resilience.

Liver function tests should therefore be interpreted as indicators of overall metabolic function rather than isolated disease markers. When considered alongside insulin sensitivity, inflammatory markers, body composition, lifestyle, and clinical history, they provide valuable insight into both liver health and broader metabolic recovery.
